Job Responsibilities

Provide skilled nursing services to patients struggling with substance use disorders.
Manage detoxification, ongoing assessment, evaluations, triage, harm reduction teaching, stabilization, and monitoring of clients/patients.
Develop goal-oriented treatment plans with clients/patients.
Collaborate with primary care providers, integrated treatment team, and community providers.
Ensure medication is properly distributed and accounted for.
Facilitate and schedule intakes, discharges, and transfers to necessary programs.
Provide nursing care to patients, adjust medications within medical direction.
Conduct assessments and develop appropriate treatment plans.
Perform ongoing management of patients receiving medications for addiction.
Regularly evaluate client/patient progress and provide crisis intervention as needed.
Submit timely clinical documentation.
Coordinate services with clients, families, and providers involved in treatment plans.
Determine medical emergencies and take appropriate action.
Provide advocacy to ensure client/patient understands rights.
Offer information, education, and support for accessing opioid treatment programs.
Assess clients/patients continuously for signs and symptoms.
Maintain clear communication and documentation of patient care data.
Provide follow-up care in person and via telephone when needed.
Stay updated on current addiction treatment trends and local resources.
Participate in cross-functional teams and performance improvement initiatives.
Perform other related duties as required.

Requirements

Experience: At least one year of professional nursing experience, preferably in a detox unit and/or inpatient, partial hospitalization and/or outpatient setting.

Education: Must be a Registered Nurse (RN) licensed in Texas.
