Job Responsibilities

Direct Patient Care Technician's provision of safe and effective delivery of chronic hemodialysis therapy to patients.
Delegate tasks to direct patient care staff including LVN/LPNs, Patient Care Technicians, and Dialysis Assistants.
Assess daily patient care needs and develop appropriate patient care assignments.
Monitor patient care staff for appropriate techniques and adherence to facility procedures.
Participate in staff training and orientation of new staff as assigned.
Participate in all required staff meetings.
Ensure educational needs of patients and families regarding End Stage Renal Disease (ESRD) are met.
Provide ongoing education to patients regarding their renal disease and dialysis therapy.
Discuss and record patient education related to diet, fluid, and medication compliance.
Provide detailed education regarding adequacy measures.
Ensure transplant and modality awareness is communicated to patients.
Provide safe and effective delivery of care to patients with ESRD.
Accurately implement treatment prescriptions.
Assess patient responses to treatment and adjust the plan as needed.
Communicate problems or concerns to the Team Leader or physician.
Initiate Initial and Annual Nursing Assessment according to FMC policies.
Participate in pre-evaluation, initiation, monitoring, and termination of hemodialysis therapy.
Take appropriate interventions for changes in patient adequacy status.
Ensure accurate and complete documentation by Patient Care Technician.
Review, transcribe, and enter physician lab orders into the Medical Information System.
Ensure lab requisitions and specimens are prepared and processed correctly.
Report alert/panic and abnormal lab results to the physician.
Maintain shift operation in a safe, efficient, and effective manner.
Collaborate with healthcare team members to interpret and coordinate patient care.
Administer prescribed medications and document medical justification when necessary.
Ensure a clean, safe, and sanitary environment in the dialysis facility.
