Job Responsibilities
- Ensure the delivery of high-quality care to our long-term care residents.
- Conduct thorough initial interviews and assessments for new residents.
- Develop individualized care plans focused on both short-term and long-term goals.
- Lead meetings with multidisciplinary teams, caregivers, and families to coordinate resident care.
- Work with specialists to optimize resident outcomes.
- Guide nursing staff, communicate effectively at all levels, and conduct in-service education on MDS, RAPS, and Triggers.
- Maintain compliance with the MDS/Care Planning schedule for non-Medicare residents.

- Strong working knowledge of MDS

- Knowledge of Medicare guidelines

Requirements
Experience: Minimum of 5 years of experience in a Skilled Nursing Facility as an MDS Coordinator.
