Job Responsibilities

Drive member behavior change, health coaching/lifestyle modification, and social determinants of health remediation.
Engage family members and facilitate direct collaborative provider engagement.
Serve as the individual’s single point of contact with minimal handoffs to other disciplines.
Assess four potential problem domains including medical, behavioral, social, and health systems using a holistic approach.
Identify individual barriers to health improvement and compliance through a relationship-based assessment.
Leverage individuals' motivation for change and create prioritized interventions based on individual goals.
Establish and measure outcomes based on care plan goals, clinical, functional, economic, and quality of life outcomes.
Responsible for essential activities of case management including assessment, planning, implementation, coordination, monitoring, and evaluation.

Experience working in an Integrated Behavioral Health-Medical model

Requirements

Experience: 3 years of case management, utilization management, or behavioral health case management experience.

Education: Associate or Bachelor's Degree in Nursing (for RN position); Master's degree in Behavioral Health related field (for Clinician position).

License & Certification:

Registered nurse (RN) license (current and unrestricted) for Nurse position.
Independent clinical license such as LCSW, LICSW, LMHC, LPC, or LMFT for Clinician position.
