Pay: $42.00 - $44.00 an hour; full salary range $34.20 - $55.70 an hour; bonus target 10%

Drive member behavior change, health coach/lifestyle modification, social determinants of health remediation.
Engage in intensive coordinated transitions of care and family member involvement.
Serve as the single point of contact for individuals with minimal handoff to other disciplines.
Assess medical, behavioral, social, and health system issues using a holistic approach.
Identify barriers to health improvement and compliance using a relationship-based assessment approach.
Leverage individual motivation for change to create prioritized intervention plans.
Establish and measure outcomes based on care plan goals, clinical performance, and quality of life metrics.
Conduct essential case management activities including assessment, planning, implementation, coordination, monitoring, and evaluation.

Knowledge of health insurance industry trends, technology, and contractual arrangements.
Proficiency in Microsoft Office and general computer skills.
Strong oral, written, and interpersonal communication skills.
Ability to interpret policies and make clinical decisions effectively.
Strong organizational and time management skills.
Critical thinking and decision-making ability.
Patient-focused problem-solving skills.

Experience: 3 years of case management, utilization management, disease management, or behavioral health case management experience.

Education: Associate or Bachelor’s Degree in Nursing for Nurse role; Master’s degree in Behavioral Health for Clinician role.

License & Certification:

Registered Nurse (RN) license (for Nurse role), master's-level independent clinical license (for Clinician role).
