Job Responsibilities
- Collaborate with care team (IDT) and other professionals such as the Social Worker, Chaplain, Volunteers to create Plan of Care.
- Schedule in-home visits to evaluate patient's condition and coordinate Plan of Care.
- Assess, monitor, and keep a record of patient's condition.
- Notify doctor of change in patient's condition when deemed necessary.
- Provide education to patient, family, and care team.
- Maintain precise records and documentation in a timely manner.
Required Skills

- Must possess a current RN license.
- 1 - 3 years Hospice nursing experience preferred but not necessary.
- Multi-task and time management skills.
- Excellent communication skills, both verbal and written.
- Naturally compassionate and end of life understanding.
- Exceptional interpersonal skills.
- Accurate and timely charting.
- Team-oriented.
- Ability to handle intense situations.
- Current CPR certificate.
- Reliable transportation.
- Ability to navigate a computer/computer program and learn new software as necessary.
