Job Responsibilities
- Provide in-home and facility-based hospice care through assessment, diagnosis, and treatment.
- Conduct physical exams and face-to-face visits to determine hospice eligibility.
- Develop and manage individualized care plans in collaboration with the interdisciplinary team.
- Prescribe medications and treatments as appropriate.
- Educate and support patients and families on symptom management and disease progression.
- Coordinate care with physicians, nurses, social workers, and spiritual care providers.
- Maintain accurate clinical documentation in compliance with regulatory standards.
