Job Responsibilities
- Lead and collaborate in the management and delivery of patient care plans.
- Support patients in Elmwood Park, Berwyn and surrounding areas.
- Meet critical deadlines for charting.
Required Skills
Core Skills
- RN license in the state of employment
- Clinical competence to deliver quality patient care
- Time and schedule self-management skills
- Experience with tablets, mobile phones, and EMR software
Preferred Skills
- Experience in home health nursing
Requirements
Experience: One year of RN experience
Education: Graduate from an approved school of practical nursing
License & Certification:
- RN license

- Ability to spend approximately 20% of the day driving to/from patient locations
