Pay: $19.86 to $38.85 per hour

Shift: Monday-Friday, 9:30am - 6:00pm

Conduct a high volume of outbound calls to assist members, evaluate current health status, and discuss non-compliance to medication(s).
Schedule pharmacy telephonic appointments and obtain important follow up information from providers.
Answer inbound calls from members and assist them with their inquiries.
Identify patients with missing measures and work to close the measures through education, counseling, appointment setting, and other means.
Follow system scripting and validate member demographic information.
Document the provider or member's record with accurate information obtained on the call.
Maintain education on HEDIS/STARs standards and guidelines.

Multitask and perform all other related duties as assigned.

Current, unrestricted LVN/LPN license
3+ years of clinical experience as a LVN or LPN
3+ years of experience in the health industry and/or in a large complex matrixed organization
Intermediate level of proficiency with MS Excel, Word, and PowerPoint
Intermediate level of proficiency with SharePoint
