Position: Admission Nurse
Job Type: Full Time
Pay: $83,000 - $94,000 per year
Shift: Monday, Tuesday & Saturday (9am-9pm), 3 - 12hr shifts a week

Job Responsibilities
- Review clinical referral information and respond to requests and inquiries as appropriate.
- Collaborate with the Clinical Director regarding the eligibility and appropriateness of clients for hospice services.
- Provide information, pre-assessment, and admission services to patients who have been referred to the hospice, conducting pre-assessments visits in the home or healthcare facility as necessary.
- Communicate with the Intake Department, the referring physician, and the Hospice Medical Director to assure appropriateness of each referral and that the necessary certifications are obtained.
- Apply nursing principles and techniques in the treatment of patients.
- Provide nursing care, emotional support, and education to patients and families.
- Encourage patients and family members to participate in the patient's plan of care to meet established goals.
- Participate in On-Call duties as assigned by the Clinical Director and Program Director.
Required Skills
Core Skills
- Current unencumbered license as a Registered Nurse
- Computer proficiency including background in EMR
- Current CPR certification
Preferred Skills
- Bachelor's degree in nursing or health care
- Minimum one (1) year experience as an RN in a medical surgical/acute care setting
- Hospice/home health experience
