Conduct assessments, including comprehensive annual wellness exams on patients in-home and virtually
Counsel and educate patients and families about benefits and programs for healthier living
Document chief complaints, diagnoses, medical history, examinations, medications, allergies, assessments, and plans
Coordinate care with primary care providers, specialists, and ancillary services
Complete documentation and paperwork in a timely manner
Maintain quality of care standards as defined by the practice
Deliver evidence-based, timely care to reduce avoidable hospitalizations and enhance patient satisfaction
Prescribe medications, order tests, and collaborate with patients' Monogram physicians
Facilitate closing gaps in care by educating patients about preventive monitoring
Assist patients in enrolling to access educational videos
Participate in integrated care team meetings
Make assessments of patients' health status
Develop and implement treatment plans consistent with appropriate care plans
Follow up and evaluate patients' status
Perform other duties as assigned

Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license
Board certified for appropriate licensure (NP: ANCC/AANP; PA: NCCPA)
Current and unrestricted DEA certificate
Ability to work autonomously
Proficiency with medical instruments and equipment
Knowledge of computer-based data management programs and medical records
Effective communication in verbal and written form

Bilingual (English/Spanish) highly preferred
2 or more years of direct patient care experience
Managed Care/IPA/Health Plan experience
Experience conducting annual wellness visits or similar comprehensive visits virtually or in-home

Experience: 2 or more years of direct patient care required
