Job Responsibilities

  • Conduct assessments, including comprehensive annual wellness exams on patients in-home and virtually
  • Counsel and educate patients and families about benefits and programs for healthier living
  • Document chief complaints, diagnoses, medical history, examinations, medications, allergies, assessments, and plans
  • Coordinate care with primary care providers, specialists, and ancillary services
  • Complete documentation and paperwork in a timely manner
  • Maintain quality of care standards as defined by the practice
  • Deliver evidence-based, timely care to reduce avoidable hospitalizations and enhance patient satisfaction
  • Prescribe medications, order tests, and collaborate with patients' Monogram physicians
  • Facilitate closing gaps in care by educating patients about preventive monitoring
  • Assist patients in enrolling to access educational videos
  • Participate in integrated care team meetings
  • Make assessments of patients' health status
  • Develop and implement treatment plans consistent with appropriate care plans
  • Follow up and evaluate patients' status
  • Perform other duties as assigned

Required Skills

Core Skills

  • Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license
  • Board certified for appropriate licensure (NP: ANCC/AANP; PA: NCCPA)
  • Current and unrestricted DEA certificate
  • Ability to work autonomously
  • Proficiency with medical instruments and equipment
  • Knowledge of computer-based data management programs and medical records
  • Effective communication in verbal and written form

Preferred Skills

  • Bilingual (English/Spanish) highly preferred
  • 2 or more years of direct patient care experience
  • Managed Care/IPA/Health Plan experience
  • Experience conducting annual wellness visits or similar comprehensive visits virtually or in-home

Education:

License & Certification:

  • Active and unrestricted Registered Nurse license
  • Board certification for Nurse Practitioner or Physician Assistant
  • Current and unrestricted DEA certificate
