Job Responsibilities
- Collaboratively work as an active member of the interprofessional team to provide therapeutic patient and family centered care.
- Systematically and continuously collect and assess data related to patient health status to develop, execute and evaluate the plan of care.
- Utilize evidence to drive clinical decisions and practice related to individual and population health.
- Establish goals and strategies for meeting care coordination needs across settings with the patient, family, and/or other care providers.
- Consider factors related to safety, effectiveness, and fiscal accountability in planning and delivering care.
- Delegate and supervise care in accordance with the WI Nurse Practice.
- Participate in continuous quality improvement initiatives and assume responsibility for professional development.
- Perform/direct ACLS and independent rhythm interpretation and defibrillation.
- Administer advanced lifesaving medications and conduct independent, templated RN-only visits for patient assessment based on established protocols.
- Provide specialty patient education where certification is required.
- Assist with complex procedures and advanced wound therapy.
- Manage central lines and perform device interrogation with resultant action.
- Coordinate patient care, provide direct patient care, patient education, telephone triage, eVisits, and assist with minor procedures.

Requirements
Experience: At least 1 year of relevant nursing experience preferred.
Education: Bachelor's Degree in Nursing preferred.
License & Certification:
- Current state of Wisconsin Registered Nurse License or Multi-state Nursing License from a participating state in the NLC
- AHA Basic Life Support for the Healthcare Provider (BLS) or equivalent required within 90 days of hire.
