Implements and monitors patient care plans.
Monitors, records, and communicates patient condition as appropriate.
Serves as a primary coordinator of all disciplines for well-coordinated patient care.
Notes and carries out physician and nursing orders.
Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
Conducts systematic assessments of patients, including physiological, psychological, sociocultural, spiritual, economic, and lifestyle factors.
Performs initial assessments, re-assessments, and focused assessments based on patient condition.
Identifies problems based on clinical judgments regarding patient responses to health conditions.
Plans and sets measurable and achievable short- and long-range goals in collaboration with patients and families.
Establishes daily goals with the patient and family.
Formulates a plan of care and ensures continuity of care during hospitalization and discharge.
Documents nursing care in healthcare records.
Evaluates patient status and effectiveness of nursing care continuously.
Coordinates care to manage the sequence, timing, and effectiveness of patient care from admission to discharge.
Delegates and supervises nursing tasks appropriately.
Prioritizes patient care needs.
