Job Responsibilities

Collect pertinent data and information relative to the healthcare consumer's health or situation.
Analyze assessment data to determine actual and potential diagnoses or issues.
Identify expected outcomes for a plan individualized to the healthcare consumer or situation.
Develop and modify a plan of care that prescribes strategies and alternatives to attain expected outcomes.
Implement the identified plan.
Coordinate care delivery and transitions in care.
Employ strategies to promote health and a safe environment.
Evaluate progress toward attainment of outcomes.
Practice ethically and in congruence with cultural diversity and inclusion principles.
Communicate effectively in all areas of practice.
Collaborate with healthcare consumers and key stakeholders in the conduct of nursing practice.
Lead within the professional practice setting and the profession.

Requirements

Experience: Nursing experience as a Registered Nurse; encouraged to advance through the SPIRE clinical advancement program.

Education: Graduation from an accredited school of nursing, BSN preferred.

License & Certification:

Licensed and currently registered nurse in the State of Connecticut
Current BCLS certification or must obtain within one month of hire
