Analyze the assessment data to determine actual and potential diagnoses or issues.
Identify expected outcomes for a plan individualized to the healthcare consumer or the situation.
Develop and modify plans of care that prescribe strategies and alternatives to attain expected outcomes.
Implement identified plans.
Coordinate care delivery and transitions in care.
