Job Responsibilities
- Collects pertinent data and information related to the healthcare consumer's health or situation.
- Analyzes assessment data to determine actual and potential diagnoses or issues.
- Identifies expected outcomes for a plan individualized to the healthcare consumer or the situation.
- Develops and modifies a plan of care that prescribes strategies and alternatives to attain expected outcomes.
- Implements the identified plan.
- Coordinates care delivery and transitions in care.
- Employs strategies to promote health and a safe environment.
- Evaluates progress toward the attainment of outcomes.
- Practices ethically and communicates effectively in all areas of practice.

Requirements
Experience: A Minimum of 3 years Intensive Care Experience Required
Education: Graduation from an accredited school of nursing. BSN preferred.
License & Certification:
- Licensed and a currently registered nurse in the State of Connecticut
- Current BCLS certification or must obtain within one month of hire
