Job Responsibilities

Provide individualized and comprehensive care to reduce barriers and ensure prompt access to care.
Perform clinical assessments and educate on disease process, plan of care, and treatment modalities.
Manage symptoms to support patients through the complexity of their care continuum.
Collaborate and communicate effectively with the multidisciplinary team to coordinate patient care.
Initiate referrals to appropriate interdisciplinary teams for physical, psycho-social, and spiritual support.
Provide coordination of care and support for patients, families, and caregivers throughout the continuum of care.
Educate patients, caregivers, and families on disease process, plan of care, treatment, and survivorship.
Communicate effectively with patients, caregivers, families, and multidisciplinary healthcare teams.
Gather pertinent patient medical history and review presenting symptoms for nursing assessments.
Demonstrate professionalism in the workplace with patients, families, healthcare team, and community.
Provide excellent customer service resulting in patient satisfaction with care.
Promote a patient-and family-centered care environment for ethical decision making and advocacy for patients.
Welcome patients and family members warmly and introduce self and multidisciplinary team members.
Communicate time for appointments, procedures, and potential delays to patients.
Prioritize incoming phone messages and triage according to clinical needs timely.
Return patient calls within the same day and use positive service recovery techniques.
Respond to patient messages and/or requests within 24 hours.

Required Skills

Core Skills

BLS Certification from American Heart Association
EMR documentation skills
Patient education and interventions experience
Symptom management experience
Nursing assessment skills

Preferred Skills

Bachelor of Science in Nursing (BSN)
3 years of experience as an RN in an inpatient or outpatient setting

Requirements

Experience: 3 years of experience as an RN in an inpatient or outpatient setting

Education: ADN Completion of accredited RN program

License & Certification:

AZ RN license
BLS Certification
