Job Responsibilities
- Provide individualized and comprehensive care to reduce barriers and coordinate diagnostic and treatment efforts.
- Ensure prompt access to care by applying the nursing process and performing clinical assessments.
- Educate patients on disease processes, plans of care, treatment modalities, and symptom management.
- Support patients through the complexity of their care continuum.
- Collaborate and communicate effectively with the multidisciplinary team to ensure continuity of care.
- Create a comfortable and exceptional patient experience.
- Initiate referrals to appropriate interdisciplinary teams for physical, psycho-social, and spiritual support.
- Provide coordination of care and support for patients, families, and caregivers throughout the continuum of care.
- Effectively communicate with patients, caregivers, families, and multidisciplinary healthcare teams.
- Gather pertinent patient medical history and review presenting symptoms for nursing assessments.
- Demonstrate professionalism in the workplace with patients, families, healthcare teams, and the community.
- Provide excellent customer service resulting in patient satisfaction.
- Promote a patient-and family-centered care environment for ethical decision-making and advocacy.
- Provide a warm welcome to patients and family members with introductions to self and multidisciplinary team members.
- Communicate appointment times, procedures, and potential delays.
- Prioritize incoming phone messages and triage according to clinical needs in a timely manner.
- Return calls within the same day and use positive service recovery techniques.
- Return patient messages and/or requests within 24 hours.
Required Skills
Core Skills
- Registered Nurse (RN) with a minimum of 3 years of experience in an inpatient or outpatient setting.
- Knowledge of patient education/interventions and symptom management.
- Proficient in computer skills, EMR documentation, and Record/Verify systems.
Preferred Skills
- Bachelor of Science in Nursing (BSN)
- American Heart Association Basic Life Support (BLS) certification.
Requirements
Experience: 3 years of experience as a RN in an inpatient or outpatient setting.
Education: Completion of an accredited RN program.
License & Certification:
- AZ RN license
- American Heart Association BLS certification.
