Job Responsibilities
- Provide individualized and comprehensive care to reduce barriers, coordinate diagnostic and treatment efforts, and ensure prompt access to care.
- Perform clinical assessments and educate on disease processes, plans of care, treatment modalities, and symptom management.
- Collaborate and communicate effectively with the multidisciplinary team to coordinate patient care and ensure continuity of care.
- Initiate referrals to appropriate interdisciplinary teams for physical, psycho-social, and spiritual support.
- Provide care coordination and support for patients, families, and caregivers throughout the continuum of care.
- Educate patients, caregivers, and families on the disease process, plan of care, treatment, symptom management, and survivorship.
- Communicate effectively with patients, caregivers, families, and multidisciplinary healthcare teams.
- Conduct nursing assessments by gathering pertinent medical history and reviewing presenting symptoms.
- Demonstrate professionalism in the workplace with patients, families, the healthcare team, and the community.
- Provide excellent customer service resulting in patient satisfaction.
- Promote a patient-and family-centered care environment for ethical decision making and advocacy.
- Welcome patients and family members with 2.2 introductions to self and multidisciplinary team members.
- Communicate time for appointments, procedures, and potential delays.
- Prioritize incoming phone messages and triage according to clinical needs in a timely manner.
- Return calls within the same day of receipt.
- Use positive service recovery techniques.
- Return patient messages and/or requests within 24 hours.

Requirements
Experience: 3 years of experience as a RN in an inpatient or outpatient setting.
Education: ADN Completion of accredited RN program
