Job Responsibilities

  • Perform assessment and plan patient care.
  • Evaluate the patient assessment, planning and intervention process.
  • Demonstrate professional and personal development.
  • Deliver direct patient care appropriate to the age of patient served.
  • Assess and diagnose wounds, ostomies and continence disorders.
  • Develop and implement treatment plans.
  • Monitor and evaluate patient progress.
  • Provide patient education on self-care and disease management.
  • Collaborate with other healthcare professionals to ensure holistic care.
  • Stay up-to-date with the latest research and technology in wound, ostomy and continence care.
  • Document patient progress and treatment plans accurately and thoroughly.
  • Provide emotional support to patients and their families.
  • Manage and maintain specialized equipment and supplies.
  • Advocate for the needs and rights of patients with wounds, ostomy and continence disorders.
  • Manage complex wounds, ostomies and continence issues.

Required Skills

Core Skills

  • Ability to work independently.
  • Addresses wound care consults timely.
  • Ability to teach staff on wound care, staging, & documentation.
  • Professionally interact with patients.
  • Ability to teach patients how to manage their wound care.
  • Accurately assess patient skin integrity.
  • Prevent skin breakdown in hospitalized patients.
  • Accurately stage and assess wounds.
  • Microsoft Office skills (Word, Excel, PowerPoint).

Preferred Skills

  • Wound Care Certification.
  • Two years of experience in wound, ostomy and continence care.
  • Educational and tuition assistance.
  • Professional development opportunities.

Requirements

Experience: One to two years related experience and/or training preferred.

Education: Graduate of a Program in Discipline - Required, Bachelor's Degree preferred.

License & Certification:

  • Current licensure by WV State Board of Nursing.
  • Basic Life Support (BLS) - to be obtained within 6 months.
