Job Responsibilities
- Implements and monitors patient care plans.
- Monitors, records, and communicates patient condition as appropriate.
- Serves as a primary coordinator of all disciplines for well-coordinated patient care.
- Notes and carries out physician and nursing orders.
- Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
- Conducts initial and re-assessments and focused assessments based on patient condition, setting, and population.
- Identifies problems based on clinical judgment about the patient’s response to actual or potential health conditions or needs.
- Plans and sets measurable and achievable short- and long-range goals in collaboration with the patient and family.
- Establishes daily goals with the patient and family.
- Formulates a plan of care and documents assessment data in the patient’s care plan.
- Implements nursing care according to the care plan and ensures continuity of care.
- Documents patient care in the healthcare record.
- Evaluates patient status and nursing care effectiveness continuously and modifies care plan as needed.
- Coordinates care with patients, families, and/or healthcare team members from admission to discharge.
- Delegates and supervises tasks as necessary.
- Prioritizes tasks effectively.

Requirements
Experience: None required
Education: Graduation from an accredited School of Nursing
License & Certification:
- Valid current Arizona Nursing License or compact state license and ability to become Arizona Licensed in 30 days
- Basic Life Support (BLS) - American Heart Association
