Job Responsibilities
- Provide nursing assessment and support for expectant families and those with infants until their first birthday.
- Follow home visiting scheduling protocol, including productivity requirements as outlined by the supervisor.
- Conduct visits in the home, via telephone, or virtually.
- Document client activities, assessments, and visits in real-time using a computerized record system.
- Ensure nursing progress notes conform to MHMR Tarrant standards and are submitted within established timelines.
- Conduct health and physical assessments of mothers and babies to determine well-being.
- Assess family needs in 12 domains of physical and psychosocial well-being, including depression, substance abuse, and domestic violence.
- Exercise clinical skills in appraising situations and making decisions.
- Establish trusting relationships with families during home visits and engage them in support plans.
- Apply working knowledge of parent-child interaction, child-maternal health, and child development to help improve parenting skills.
- Conduct recruitment and outreach activities to increase community awareness of perinatal connections supports and services.
- Assist with accessing eligible referrals from partner hospitals, clinics, and community partners.
- Participate in regularly scheduled team and supervision meetings.
- Engage in workgroups, committees, and coalitions as assigned.

Preferred Skills
- Bachelor’s degree in nursing or advanced degrees (MSN, MPH).
- Certification as lactation educator or consultant.
- Spanish-speaking.
Requirements
Experience: 1 year clinical experience related to domestic violence, substance abuse, maternal depression, or child health.
Education: Associate’s Degree in Nursing.
License & Certification:
- Current Texas Registered Nurse license
