Implements and monitors patient care plans.
Monitors, records, and communicates patient condition as appropriate.
Serves as a primary coordinator of all disciplines for well-coordinated patient care.
Notes and carries out physician and nursing orders.
Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
Conducts systematic and dynamic assessments to collect and analyze data about patients.
Identifies problems based on clinical judgments about patients’ responses.
Establishes measurable and achievable goals in collaboration with patients and families.
Implements nursing care according to the care plan to ensure continuity of care.
Documents care in the patient’s healthcare record.
Evaluates the patient’s status and the effectiveness of nursing care.
Coordinates care with patients, families, and healthcare team members.
Delegates and supervises care activities.
