Coordinate clinically complex patient care across a continuum.
Ensure and facilitate quality clinical and cost outcomes.
Negotiate, procure, and coordinate appropriate services and resources needed by patients.
Intervene to address and resolve issues/concerns at key points.
Ensure appropriate sequencing of treatment goals.
Implement a comprehensive discharge plan.
Demonstrate knowledge and skills necessary to provide case management services to the patient population experiencing rehabilitation needs.

Current, valid, and active license to practice as a Registered Nurse, MSW, or Licensed Social Worker in the state of employment.
Current BLS certification from a Vibra-approved vendor.
Strong clinical assessment and process skills.
Ability to make independent decisions when circumstances warrant.
Outstanding written and oral communication skills.
Organizational, prioritizing, and analytical skills.

One (1) year of case management experience.
Previous LTAC experience.
CCM or ACM certification.
Working knowledge of computer and software applications used in job functions.
Knowledgeable in utilization review, medical terminology, and healthcare delivery models.

Experience: One (1) year of case management experience preferred.
