Job Responsibilities
- Manage a caseload of ambulatory patients post-discharge.
- Support optimal recovery for patients.
- Prevent hospital readmissions.
- Reduce cost of care across the healthcare continuum.
- Interact with members of the care team to solve current situations.

Requirements
Experience: One (1) year of clinical experience required
Education: Graduate of a school of Nursing program approved and accredited by the Alabama State Board of Nursing
License & Certification:
- Currently licensed or eligible to be licensed as a Registered Nurse (RN) by the Alabama Board of Nursing
- Certified in Basic Life Support (BLS) from the American Heart Association (AHA)
- Advanced Cardiac Life Support (ACLS) certification may be required depending on the unit/department
- Pediatric Advanced Life Support (PALS) certification may be required depending on the unit/department
