Job Responsibilities
- Manage the resident care team and coordinate care and service delivery to residents.
- Liaison with hospital personnel, physicians, community organizations, and health-related agencies.
- Oversee and implement resident care programs and ensure regulatory compliance.
- Model core company values and ensure all team members exemplify them.
- Promote health and wellness by collaborating with care providers.
- Ensure services are monitored and comply with Federal and State policies.
- Support administration in referral development and achieving occupancy goals.
- Perform routine resident care rounds to meet individual care needs.
- Complete assessments on all residents as required.
- Track, trend, and report resident care quality data.
- Recommend modifications to staffing plans for optimal resident care.
- Deliver direct resident care and supervise care associates.
- Coordinate departmental schedules and ensure adequate staffing.
- Interview, hire, and monitor performance of care associates.
- Conduct orientation and oversight of continuing education programs.
- Accountability for care team associates through coaching or corrective actions.
- Provide education and training on resident care policies and regulations.
- Oversight of the community medication management program.
- Monitor changes in residents' health and communicate with relevant parties.
- Lead the care team to achieve outcome goals.
- Maintain a safe working environment.
Required Skills
Core Skills
- Strong communication skills, both verbal and written.
- Knowledge of infection control practices.
- Ability to make decisions in stressful situations.
- Operational knowledge of nursing management including hiring and coaching.
- Ability to assess resident needs and develop care plans.
Preferred Skills
- Experience with specialized memory care programs.
- Certifications in CPR and first aid.
- Proficiency in basic office equipment.
Requirements
Experience: Minimum of two (2) years’ experience providing resident care to the elderly.
Education: Minimum of two (2) years’ college in human services or healthcare field.
License & Certification:
- Unencumbered state license as a professional Registered Nurse (RN) or Licensed Practical Nurse (LPN).
- Certified in CPR and first aid.
Schedule:
- Ability to work weekends, evenings, and flexible hours, including on-call responsibilities.
