Manage the resident care team and coordinate care and service delivery to residents.
Liaison with hospital personnel, physicians, community organizations, and health-related agencies.
Oversee and implement resident care programs and ensure regulatory compliance.
Model core company values and ensure all team members exemplify them.
Promote health and wellness by collaborating with care providers.
Ensure services are monitored and comply with Federal and State policies.
Support administration in referral development and achieving occupancy goals.
Perform routine resident care rounds to meet individual care needs.
Complete assessments on all residents as required.
Track, trend, and report resident care quality data.
Recommend modifications to staffing plans for optimal resident care.
Deliver direct resident care and supervise care associates.
Coordinate departmental schedules and ensure adequate staffing.
Interview, hire, and monitor performance of care associates.
Conduct orientation and oversight of continuing education programs.
Accountability for care team associates through coaching or corrective actions.
Provide education and training on resident care policies and regulations.
Oversight of the community medication management program.
Monitor changes in residents' health and communicate with relevant parties.
Lead the care team to achieve outcome goals.
Maintain a safe working environment.

Experience: Minimum of two (2) years’ experience providing resident care to the elderly.

Education: Minimum of two (2) years’ college in human services or healthcare field.

License & Certification:

Unencumbered state license as a professional Registered Nurse (RN) or Licensed Practical Nurse (LPN).
Certified in CPR and first aid.

Schedule:

Ability to work weekends, evenings, and flexible hours, including on-call responsibilities.
