Job Responsibilities
- Provide comprehensive care coordination for assigned patients.
- Assess patient plans of care and develop, implement, monitor, and document resource utilization and patient progress.
- Manage individual patients across the health care continuum to achieve optimal clinical, financial, operational, and satisfaction outcomes.
- Lead process improvement activities for populations of patients.
- Collaboratively develop and manage interdisciplinary patient discharge plans.
- Communicate the discharge plan effectively across the continuum of care.
- Evaluate medical necessity and appropriateness of care to optimize patient outcomes.
- Assess patient admissions and continued stay using standard criteria.
- Identify and resolve issues that may delay patient discharge.
- Establish collaborative relationships with physicians, payers, and healthcare team members.
- Collect and communicate pertinent information to fulfill utilization and regulatory requirements.
- Educate internal healthcare team members about case management and managed care concepts.
- May supervise other staff.

Requirements
Experience: 3-5 years clinical experience
Education: Bachelor's degree in case management or health care
License & Certification:
- Current Registered Nurse (R.N.) license
- Basic Life Support (BLS) certification for acute care settings
Schedule:
- Flexible hours and rotating call after hours for acute care settings
