Coordinate the patient's care to increase the efficiency and quality of care delivered.
Act as a patient advocate, providing supportive and personal care.
Serve as a liaison to ensure patients and families receive necessary resources and services.
Provide language specific education, psychosocial support, and necessary resources for newly diagnosed patients.
Educate new and existing patients and families regarding diagnosis, treatments, and follow-up.
Coordinate new patient referrals, scheduling exams, procedures, and appointments.
Identify barriers to care and refer patients to necessary resources.
Conduct service recovery as necessary and responsible for patient satisfaction.

Experience: Nursing experience worked autonomously and independently

Education: BSN degree required, MSN preferred
