Job Responsibilities

Maintain a high performing hospice program including managing, overseeing, and directing patient care activities to ensure quality patient care.
Teach, evaluate, and develop associates while ensuring adherence to company policies and procedures.
Ensure compliance with local, state, and federal regulations and regulatory agencies.
Oversee and manage rescheduled, declined, missed, and reassigned visit requests.
Make decisions regarding priority and staffing based on clinical judgment.
Respond to phone referrals/inquiries in a timely, professional, and compassionate manner.
Triage medical concerns within scope of practice.
Prepare for, serve on, and facilitate the Interdisciplinary Group (IDG), including presiding over meetings.
Present patients for review, coordinate minutes, and review patient charts for any changes in condition or care plan.
Solicit input from various care team members.

Required Skills

Core Skills

Direct patient care experience
Knowledge of accepted hospice principles and practices
Compliance with industry standards, regulations, and best practices
Strong communication and interpersonal skills
Ability to advocate for hospice patients and families

Preferred Skills

Bachelor's degree in nursing
Experience in hospice or home-based care setting

Requirements

Experience: At least three years of direct patient care and two years in hospice or home-based care setting.

Education: Graduate of accredited school of nursing; bachelor’s degree in nursing preferred.

License & Certification:

Current Registered Nurse licensure in state of residence
