Job Responsibilities

Maintain a high performing hospice program including managing, overseeing, and directing patient care activities.
Ensure the delivery of quality patient care.
Teach, evaluate, and develop associates.
Ensure adherence to company policies and procedures.
Ensure compliance with local, state, and federal regulations and regulatory agencies.
Oversee and manage rescheduled, declined, missed, and reassigned visit requests.
Make decisions as to priority and staffing based on clinical judgment.
Respond to phone referrals/inquiries in a timely, professional, and compassionate manner.
Triage medical concerns within scope of practice.
Prepare for, serve on, and facilitate the Interdisciplinary Group (IDG).
Preside over IDG, present patients for review, and coordinate minutes.
Review patient charts to note and address any changes to patient condition and/or the plan of care.
Solicit input from various care team members.

Required Skills

Core Skills

Direct patient care experience
Knowledge of hospice principles and practices
Compliance with industry standards, regulations, and best practices
Ability to advocate for hospice patients and families

Preferred Skills

Bachelor's degree in nursing

Requirements

Experience: At least three years of direct patient care and two years in hospice or home-based care setting

Education: Graduate of accredited school of nursing; bachelor's degree in nursing preferred

License & Certification:

Current Registered Nurse licensure in state of residence
