Maintain a high performing hospice program.
Manage, oversee, and direct patient care activities to ensure quality delivery of patient care.
Teach, evaluate, and develop associates.
Ensure adherence to company policies and procedures, and compliance with local, state, and federal regulations.
Oversee and manage rescheduled, declined, missed, and reassigned visit requests.
Make decisions on priority and staffing based on clinical judgment.
Respond to phone referrals/inquiries in a timely, professional, and compassionate manner, including triage of medical concerns.
Prepare for, serve on, and facilitate the Interdisciplinary Group (IDG).
Preside over IDG meetings and present patients for review.
Coordinate minutes and review patient charts to note and address any changes to patient conditions and/or plans of care.
Solicit input from various care team members.

Knowledge and compliance with accepted hospice principles and practices.
Understanding of terminally ill patients and their families.
Ability to advocate and provide oversight for hospice patients from various socioeconomic backgrounds.

Experience: At least three years’ experience of direct patient care and two years’ experience in hospice or home-based care setting.

Education: Graduate of accredited school of nursing; bachelor's degree in nursing preferred.
