Job Responsibilities

  • Ensure effective utilization of health care staffing resources to meet residents' needs.
  • Manage call-outs and assign staff as necessary to address situational needs.
  • Conduct rounds at the beginning of each shift, communicating with staff about resident care and concerns.
  • Serve as a liaison for visits by physicians, assisting during resident rounds.
  • Maintain physician's visit calendar and manage resident health care documentation.
  • Collaborate with Interdisciplinary Team to implement and evaluate healthcare goals.
  • Coordinate required assessments and documentation related to Individual Program Plans.
  • Coordinate resident re-admission processes and ensure medication reconciliation.
  • Be present during mealtimes to ensure care plans are followed.
  • Ensure residents' health care plans are accurate and updated at least quarterly.
  • Maintain required documentation in electronic health records as per policies.
  • Establish effective communication with Nurse Manager, residents, families, and consultants.
  • Provide supervision and coaching of direct care personnel.
  • Respond to resident and family questions or concerns.
  • Ensure follow-up on lab and radiology reports.
  • Contact Administrator on Call for management issues when necessary.
  • Assist in the orientation of new staff as directed by DON or ADON.
  • Report allegations of abuse, neglect, and exploitation as per procedures.
  • Assist in managing the Wound Care Program and provide assessments.
  • Complete all mandatory in-services and attend staff meetings.

Preferred Skills

  • Two to five years of supervisory experience
  • Clinical nursing experience in acute, long-term care, or rehabilitation

Requirements

Experience: Three to five years clinical nursing experience and two to five years supervisory experience

Education: Graduate of an accredited school of nursing with a current R.N. license; BSN preferred

License & Certification:

  • Current R.N. license in the State of Delaware
  • CPR certification
