Implements and monitors patient care plans.
Monitors, records, and communicates patient condition as appropriate.
Serves as a primary coordinator of all disciplines for well-coordinated patient care.
Notes and carries out physician and nursing orders.
Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
Conducts systematic assessments that include physiological and psychological data.
Identifies problems based on clinical judgment regarding the patient’s response to health conditions.
Plans measurable and achievable goals in collaboration with the patient and family.
Implements nursing care according to the care plan.
Documents care in the patient’s record to ensure continuity.
Ensures patient safety and provides medication administration.
Educates patients and families.
Evaluates patient status and modifies care plans as needed.
Coordinates care management from admission to discharge.
