Job Responsibilities
- Participates in daily morning meetings.
- Provides medication management and planning/coordination, including injectable medications, to consumers in the community.
- Maintains and updates the monthly IM calendar.
- Administers IM's and prepares consumers' oral medications for weekly delivery by the clinical team.
- Provides staff education to maintain familiarity with program policies and procedures regarding medication management and secure record storage.
- Coordinates with Team prescribers and pharmacy to ensure consumers receive medications and refills.
- Transcribes orders and updates Medication Administration Records.
- Logs in medication deliveries and completes toxicology request forms.
- Monitors vital signs and side effects of medications and reports findings to the Team.
- Completes annual nursing assessments and daily progress notes.
- Assesses physical health needs and makes appropriate referrals to and collaborates with community physicians.
- Performs other duties, consistent with the goals and objectives of the program, as directed by the Team Leader.
