Job Responsibilities
- Rounding and managing the clinical care of the population at the assigned skilled nursing facilities.
- Rounding daily and prescribing medications and treatments for residents residing at SNFs.
- Providing high quality, patient-centered, comprehensive primary care services to patients residing in SNFs.
- Timely documentation of all patient care/encounters.
- Available for phone/secure messaging consultations with facility nursing staff.
- Completing provider documentation in Theoria's proprietary electronic health record (EHR) system, ChartEasy™.
- Being on-site on time for in-person scheduled shifts.
- Addressing acute/emergent issues that arise during rounding time.
