Job Responsibilities

Round and manage the clinical care of the population at the assigned SNF.
Prescribe medications and treatments for residents at SNFs.
Provide high-quality, patient-centered primary care services to patients residing in SNFs.
Timely documentation of all patient care/encounters.
Available for phone/secure messaging consultations with facility nursing staff.
Complete documentation in Theoria's electronic health record system, ChartEasy™.
Be on-site and on time for scheduled in-person shifts.
Address acute/emergent issues for nursing staff during rounding time.
