Pay: Salary based with monthly productivity bonuses
Shift: Monday-Friday, shifts begin around 7 AM local time, rounding from 7 AM-1 PM

Job Responsibilities
- Round and manage the clinical care of the population at the assigned skilled nursing facility (SNF)
- Provide high quality, patient-centered, comprehensive primary care services to patients at SNFs
- Rounding daily and prescribing medications and treatments for residents at SNFs
- Timely documentation of all patient care and encounters
- Available for phone/secure messaging consultations with facility nursing staff
- Complete provider documentation in Theoria's proprietary electronic health record system, ChartEasy™
- Address acute/emergent issues that arise during rounding time
