Position: Nurse Practitioner or Physician managing clinical care at Skilled Nursing Facilities
Job Type: Full-time
Pay: Up to $165,000 annually, plus monthly productivity bonuses
Shift: Monday-Friday, 7 AM-1 PM

Job Responsibilities
- Rounding and managing the clinical care of the population at the assigned SNF
- Rounding daily and prescribing medications and treatments for residents at SNFs
- Providing high quality, patient-centered, comprehensive primary care services to patients at SNFs
- Timely documentation of all patient care/encounters in the electronic health record system
- Available for phone/secure messaging consultations with facility nursing staff
- On-site attendance for scheduled shifts
