Position: Primary Care Provider at Skilled Nursing Facilities

Pay: Up to $165,000 annually and eligible for a sign-on bonus
Shift: Monday-Friday, 7 AM-1 PM

Job Responsibilities
- Manage the clinical care of the population at the assigned skilled nursing facilities (SNF).
- Rounding daily and prescribing medications and treatments for residents at SNFs.
- Provide patient-centered, comprehensive primary care services to patients residing in SNFs.
- Timely documentation of all patient care/encounters.
- Available for phone/secure messaging consultations with facility nursing staff.
- Complete provider documentation in proprietary electronic health record (EHR) system, ChartEasy™.
- Maintain on-site presence during scheduled shifts.
