Job Responsibilities
- Provide care for the geriatric population in Skilled Nursing and/or Assisted Living settings.
- Collaborate with the skilled nursing facility’s integrated care team.
- Work with Vitae’s clinical team including adult nurse practitioners, psychiatric nurse practitioners, psychologists, LCSWs, and podiatrists.
