Job Responsibilities
- Collaborate with the PSC Physician, Primary Care Physician, and palliative team members to deliver advanced illness management and palliative care.
- Take responsibility for advanced illness management from the initial assessment through discharge.
- Perform assessments including disease-specific changes, decline in function, and inadequately controlled pain or symptoms.
- Establish individualized care plans in consultation with patients, families, and team members.
- Facilitate obtaining care for patients as per the Plan of Care.
- Participate in evening/weekend call as required.
- Prescribe medications and order diagnostic tests within professional practice scope.
- Communicate clinical findings to the PSC Physician, re-assess care effectiveness regularly.
- Maintain effective working relationships with the supportive care team and external resources.
- Document clinical findings and treatment plans in patient medical records.
- Knowledgeable in Palliative Medicine Quality Measures collection and documentation.

Requirements
Experience: Minimum of two years Nurse Practitioner experience
Education: Minimum of Masters of Science in Nursing from an accredited school of nursing
License & Certification:
- Current RN license
- Current Nurse Practitioner license
- Part B Provider certification
- Federal DEA license
- State DEA license (if required)
- ACHPN certification in Hospice and Palliative Care preferred
