Job Responsibilities
- Complete the RAI process and care management process from admission to discharge.
- Work in collaboration with the Interdisciplinary Team to assess the needs of the resident.
- Provide interdisciplinary schedule for MDS assessments and care plan reviews as required by governing agencies.
- Ensure decisions for MDS assessments are made based on clinical criteria as identified in the RAI User's Manual.
- Assist with coordination and management of the daily stand up meeting.
- Comply with federal and state regulations regarding the RAI process.
- Monitor MDS and care plan documentation for all residents.
- Maintain current MDS status of assigned residents according to guidelines.
- Perform accurate data entry of resident information into MDS programs.
- Complete coding of the MDS using medical record review and interviews.
- Attend interdisciplinary team meetings and quality assurance meetings.
- Assist with identification of significant changes in residents' care.
- Prepare scheduling and notices for resident care planning conferences.
- Update knowledge related to data entry and computer technology.
- Complete electronic submission of required documentation to state database.
- Ensure completion of final MDS and submit resident assessment data.
- Assign and instruct staff in the RAI Process and related processes.
- Maintain confidentiality of necessary information.
- Perform other duties as assigned.
Required Skills
Core Skills
- Knowledge of Case-Mix and Federal Medicare PPS process
- Understanding of Medicaid reimbursement
- Knowledge of the OBRA regulations
- Knowledge of care planning process
- Experience with MDS 3.0
Preferred Skills
- Minimum of 2 years of nursing experience in a Skilled Nursing Facility
- Excellent knowledge of Quality Indicator process
Requirements
Experience: Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred
Education: Graduate of an approved Registered Nurse program and licensed in the state of practice
