Job Responsibilities

Verify and execute the plan of care as prescribed by physicians under the guidance of a Registered Nurse.
Provide essential nursing treatments and promptly address patient pain.
Assist with activities of daily living.
Educate both patients and caregivers on self-care techniques.
Handle medication administration and ensure the availability of necessary supplies.
Maintain accurate patient records.
Liaise with supervising RNs or physicians on patient assessments and any changes in conditions.
Contribute to team meetings to ensure cohesive patient care coordination.

Current Licensed Practical Nursing License
1+ years of Home Health & Hospice Experience
Ability to work independently and be self-directed
Effective verbal & written communication skills
Time management skills
Ability to use a full range of motion and lift 35lbs

Preferred Skills

Valid Driver's License (clean record) & reliable transportation
Proof of current auto insurance & registration
Valid AZ Fingerprint Clearance Card
Current CPR Certification
Negative TB Test
