Job Responsibilities
- Collaborate with the RN to fulfill the defined patient-specific Aide care plan
- Work with the care team as an extension of nursing or therapy services to ensure all patient needs are fully addressed
- Deliver hands-on personal care and supportive services to assist with safe transfers and ambulation
- Thoroughly document care delivery daily in the Electronic Medical Record system
- Contribute to a culture of caring through individual accountability and teamwork

Requirements
Experience: One year experience as a Home Health or Hospice Aide or Nursing Assistant in a hospital, nursing home, or home health agency
Education: High school diploma or equivalent preferred

- State-specific certified aide certification
- Valid state driver’s license
- Auto liability insurance
