Job Responsibilities

Ensure that team members meet and exceed all credentials required for their positions.
Assign patients and ensure compliance with all regulatory bodies within the area of responsibility.
Follow quality assurance program and utilization review in accordance with Home Care protocol.
Oversee and investigate patient and physician concerns and risk management issues, implementing changes and training where necessary.
Submit required reports such as infection control data, census, and team productivity.
Ensure clinical charts are submitted timely and accurately to promote appropriate patient care.
Perform supervisory visits with team members to ensure clinical competency.
Assist in reviewing, revising, and providing team member education regarding the Emergency Management program.
Participate with Case Manager on initial Plan of Care review, focusing on quality, utilization, and individual patient care goals.

Prior experience in adult skilled home health

Requirements

Experience: 2 years Nursing or 2 years Home Care and 1 year Supervisory Role in a related field or 6 months supervisory experience with 1 year Home Health RN visit experience

Education: Diploma, Associate's, or Bachelor's in Nursing
