Job Responsibilities

Lead assigned patient services and ensure team members meet required credentials.
Assign patients and maintain compliance with all regulatory bodies.
Ensure the quality assurance program and utilization review is followed according to Home Care protocol.
Oversee and investigate patient and physician concerns, risk management issues, and implement necessary changes and training.
Submit required reports, such as infection control data, census, staffing, and team productivity.
Ensure clinical charts are submitted timely and accurately for appropriate patient care.
Perform supervisory visits with team members to assure clinical competency.
Assist in reviewing, revising, and educating team members regarding the Emergency Management program.
Participate with Case Manager in the initial Plan of Care review focusing on quality, utilization, and patient care goals.

Requirements

Experience: 2 years nursing or 2 years home care and 1 year supervisory role in a related field or 6 months supervisory experience with 1 year home health RN visit experience

Education: Diploma, Associate's or Bachelor's in Nursing
