Job Responsibilities
- Gather necessary information from referral sources.
- Promote Agency services.
- Function as a clinical resource and customer service representative.
- Evaluate referrals for client appropriateness for home health services.
- Receive and document medical orders.
- Accept referrals for services if appropriate and fully process (or delegate) them in an accurate and timely manner.
Required Skills
Core Skills
- Registered Nurse licensure in the applicable state.
- Proficiency with Epic and/or Home Care EMR Software.
- Excellent oral and written communication skills.
- Proficiency in computer office software.
Preferred Skills
- 1-3 years prior experience in a customer service, homecare intake or admissions role.
Requirements
Experience: 2+ years of Home Health RN experience; one year of professional nursing experience in CA.
Education: Graduate of an approved Nursing education program.
License & Certification:
- Current Registered Nurse licensure
Schedule:
- Weekend rotation, one every 4 weeks
- Holiday rotation, one to two per year
