Job Responsibilities

Collaborates with providers in the delivery of patient care.
Performs triage assessment process of walk-in patients and telephone calls.
Delivers health education related to medication, diet, treatment of diagnosis, health promotion, prevention and exit health teaching.
Administers immunizations, applicable contraception refills, and medications as ordered by provider.
Performs other duties as assigned.

Required Skills

Core Skills

Current Illinois RN licensure
Current BLS for Healthcare Provider CPR certification
Community health nursing or ambulatory care experience

Preferred Skills

Bilingual

Requirements

Experience: One to two years’ experience in community health nursing or ambulatory care preferred.

Education: Current Illinois RN licensure required.

License & Certification:

BLS for Healthcare Provider CPR certification
