Job Responsibilities
- Develops, plans, implements, analyzes, and organizes clinical operations for a specific location managed.
- Conducts/delegates the assessment and reassessment of patients, including updating care plans and interpreting patient needs.
- Manages the assignment of caregivers.
- Oversees the delivery of care to all patients served by the location.
- Receives case referrals and reviews available patient information related to the case.
- Determines home health or hospice needs and ensures patients meet admission criteria.
- Assigns appropriate clinicians to a case, as needed.
- Instructs and guides clinicians to promote effective performance and delivery of quality home care services.
- Assists clinicians in establishing immediate and long-term therapeutic goals and developing patient plans of care.
- Monitors cases for compliance with regulatory agencies and third-party payer requirements.
- Ensures timely completion of audits and billing compliant with Medicare regulations.
- Coordinates communication between team members, attending physicians, and caregivers.
- Works with the Branch Director and Company Finance Department to establish revenue and budget goals.
- Participates in sales and marketing initiatives.
- Supervises all clinical employees at the location and evaluates their performance.
- Handles employee corrective action and discipline issues fairly.
- Participates in the interviewing, hiring, training, and development of direct care clinicians.
- Coaches staff and recommends in-service education programs as needed.
- Evaluates staff education needs and conducts regular staff education.
- Analyzes situations, identifies problems, and evaluates alternative courses of action.
- Reviews documentation for appropriateness of care and compliance.
- Responsible for QA/PI activities and works with Utilization Review staff for data tracking.
- Provides direct patient care on an infrequent basis and only in emergencies.
- Acts as Branch Director in their absence.
- Ensures compliance with external regulatory authorities and internal standards.
- Maintains relationships with referral and community sources and participates in professional organizations.

Requirements
Experience: Two years as a Registered Nurse in a home care, hospice or equivalent environment, with at least one-year of management experience preferred.
Education: Graduate of an accredited School of Nursing.
License & Certification:
- Current state license as a Registered Nurse
- Proof of current CPR
- Valid driver’s license and auto insurance
