Job Responsibilities

Supervise and support Clinical Care Managers across the region.
Monitor clinical documentation, outstanding orders, plans of care, and ensure timely and compliant care coordination.
Act as a clinical resource for escalated care needs and complex cases.
Step in with a small caseload when needed to ensure continuity of care.
Participate in quality improvement initiatives.
Help identify and correct clinical performance issues while promoting best practices aligned with regulatory standards.
Contribute to the hiring, onboarding, and ongoing development of Clinical Care Managers.
Support professional growth, competency assessments, and performance management in partnership with leadership.
Work closely with the Director of Nursing, branch staff, and interdisciplinary teams to drive care consistency, staff accountability, and operational alignment.
Stay current on home health regulations and requirements, ensuring the region meets standards for clinical excellence and survey preparedness.
